Lumbar spine trabecular bone score (TBS) may be an indicator for determining skeletal deterioration in people with diabetes who have high bone mineral density (BMD), according to a study published in The Journal of Clinical Endocrinology & Metabolism.
Lead researcher Jung Hee Kim, MD, of the Department of Internal Medicine at Seoul National University College of Medicine, and colleagues found that there was an inverse relationship between lumbar spine TBS and insulin resistance in people with diabetes.
TBS is a novel texture index that evaluates pixel gray-level variations in lumbar spine dual energy X-ray absorptiometry (DXA) images, and is related to bone microarchitecture independent of BMD.
The researchers sought to discover if lumbar spine TBS could be an indicator of skeletal deterioration in diabetes. The cohort study included 1,229 men and 1,529 postmenopausal women, aged older than 50 years.
The results showed that lumbar spine TBS was lower in men with diabetes than in men without diabetes (1.287 vs. 1.316; P<.001), whereas lumbar spine BMD was higher in men with diabetes (1.135 vs. 1.088 g/cm2).
Lumbar spine TBS was only lower in women with diabetes than in women without diabetes in an unadjusted model (1.333 vs. 1.353). However, women aged younger than 65 years with diabetes had lower TBS than those without diabetes, even after adjusting for covariates (P<.001).
TBS was negatively correlated with HbA1c, fasting plasma glucose, fasting insulin and homeostasis model assessment insulin resistance (HOMA-IR), but not with HOMA-beta cell function in both sexes.
The researchers believe that the inverse association between lumbar spine TBS and insulin resistance may make it an indicator for determining skeletal deterioration in patients with diabetes who have high BMD.
